無症狀黏膜下腫瘤 內視鏡治療方法多
（優活健康網記者林奐妤／綜合報導）40歲的王小姐做胃鏡及大腸鏡健康檢查時，胃鏡檢查發現胃小彎有1顆1公分左右的黏膜下腫瘤，進一步接受內視鏡超音波檢查確定。施行內視鏡黏膜下剝離術，切除腫瘤。手術過程順利，無穿孔等併發症，術後開始進食也無任何不適，術後第3天出院。
黏膜下腫瘤通常小於2公分 大部分沒有症狀
成大醫院胃腸肝膽科醫師康瑞文表示，一般熟知的胃癌及大腸癌，是由腸胃道最內的黏膜層長出來的病灶，往外的黏膜肌層、黏膜下層及肌肉層所長出來的腫瘤則統稱為黏膜下腫瘤。黏膜下腫瘤上面覆蓋有正常黏膜層組織，當腫瘤體積大到將覆蓋在上面的黏膜層頂起來時，才會在鏡檢時被發現。
當懷疑有黏膜下腫瘤時，須進一步安排內視鏡超音波確定腫瘤大小、超音波特徵及從腸胃道的哪一分層出來，來推測是脂肪瘤、囊腫或異位性胰臟等良性腫瘤，或腸胃道基質瘤、神經內分泌瘤或神經鞘瘤等惡性腫瘤。根據以往報告，鏡檢發現的黏膜下腫瘤通常<2公分，大部分沒有症狀。
可選擇以內視鏡方式切除
但是，當腫瘤逐漸長大就有可能會產生腹痛、腸胃道出血或阻塞等症狀。康瑞文醫師表示，如果仍無法判斷，就需以內視鏡超音波導引做組織取樣，決定是否切除腫瘤。
康瑞文醫師表示，現在對<2公分的黏膜下腫瘤，會優先考慮用內視鏡切除，並根據腫瘤的位置、大小，可以選擇內視鏡黏膜切除術、內視鏡黏膜下剝離術或內視鏡黏膜下隧道腫瘤切除術。相較於外科手術而言，內視鏡有保留器官、縮短住院時間及術後即可進食的優勢。
